Terms and Conditions
By joining the Warzone Boxing Club, hereinafter referred to as the CLUB, it is strongly recommended that you and all members undergo a physical examination by a doctor before using any exercise equipment or participating in any classes offered by the CLUB. The term "MEMBER" encompasses both my child and myself as active participants at the CLUB.
As the undersigned, I grant permission for the mentioned MEMBER to engage in the activities at the CLUB, releasing and waiving any liability from the CLUB, its employees, staff, coaches, agents, sponsors, and if relevant, landlords, property managers, property owners, and lessors of premises, even if such liability arises from negligence or carelessness on the part of any of the aforementioned entities.
Acknowledging the potential for serious injuries during these activities, I accept the risks on behalf of the MEMBER and agree to indemnify and hold harmless all individuals or entities listed above who might otherwise be liable due to negligence or carelessness. This waiver, release, and assumption of risk extends to my heirs and assigns.
PHOTOGRAPHIC CONSENT: I consent to the capture of photographs, images, videos, and other media of the MEMBER or any individual on the premises in connection with the MEMBER's participation at the CLUB, without expecting compensation from the CLUB or any permittee/sponsor. I authorize the use of such media for legal purposes and agree to comply with the CLUB's rules and regulations.
PARENT OR GUARDIAN CONSENT FOR TREATMENT OF MINOR: In the event of a sudden illness, accident, or injury involving the minor while under the CLUB's supervision, I authorize emergency treatment by any licensed medical provider in Arizona, California or in any other state that member is actively participating in, when neither the minor's parent(s), guardian(s), nor designated family medical provider can be reached.
In the case of death or permanent disability preventing the continuation of the membership, the buyer or buyer's estate is released from membership obligations. The MEMBER agrees to adhere to the CLUB's rules, the violation of which could lead to membership suspension or cancellation. ​
